Study visit and payment ledger

Accrued means compensation earned under the approved schedule. Expense is the reimbursement claim; mark it pending until approved. Received is money actually paid. Use separate rows when compensation and reimbursement have different due dates.

Hypothetical example: a completed visit earns $100 and approved parking costs $15. If $100 arrives, $15 remains outstanding. Record the visit payment and parking claim on separate rows so you can track each deadline.

The page code does not save or send entries to Clinical Forage. A browser may restore form values. Use Clear entries before leaving a shared device, and print when you need a copy.

Ask about a missing or incorrect payment

State the study identifier, site, activity date, expected amount, expected issue date, received amount, and the consent or schedule section you used. Ask for an itemized response.

“I completed [activity] on [date]. The payment schedule dated [date], section [section], lists [amount], due [date]. I received [amount] on [date]. Please confirm the remaining amount and expected payment date, or explain which schedule term changes the amount.”

Keep the reply with the schedule. If the date passes, follow up with the reference number and the payment contact named by the site.

Do not send card numbers, tax identifiers, or medical information in an ordinary email. Use the payment processor or site channel designated for sensitive data.
