Corrections policy and log

Identify the page, the exact issue, and the source that supports a correction. Clinical Forage will review the claim and document material changes.

Report an issue

Email corrections@clinicalforage.com. Include:

  • the page URL and heading;
  • the exact sentence, link, calculator behavior, or technical issue;
  • the correction you propose;
  • a primary source, registry record, consent document title, or agency guidance when available;
  • the date you observed the problem.

Do not send medical records, private study documents, identity numbers, laboratory reports, eligibility forms, or health information.

Review process

  1. Preserve the report and locate the published claim.
  2. Check the controlling source and its scope.
  3. Consult a qualified reviewer when the question requires clinical, legal, or tax interpretation.
  4. Correct the page, update the review date, and add a material-correction note when the error could change a reader’s decision.
  5. Update tools, internal links, and redirects affected by the correction.

Removed stale recruitment badges and thin insight pages. Corrected payment accrual and completion-incentive guidance. Expanded consent as an ongoing process, withdrawal choices, HIPAA authorization, research-only testing, incidental findings, and serious-adverse-event terminology. Added primary sources, author profiles, source-check dates, and correction paths.

Quiet edits

Spelling, punctuation, broken layout, and wording changes that do not alter meaning may be corrected without a log entry. Material factual changes receive an entry.
